Visiting Our Clinic

If this is your first visit, please arrive 10 minutes early to fill out our New Patient Form in our office.

If you prefer, you can download and print the forms in advance or complete them online and send them to our office.

New Patient Form

*Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

Your dentist will begin by reviewing your medical and dental history and completing a thorough dental exam. Your exam will typically include an in-depth examination of your gums, teeth, bite, and jaw, an oral cancer screening, and any required x-rays.

Treatment Plan

Based on your history and examination, your dentist will develop a custom long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you may have about your dental health and treatment options.

• How can I book an appointment?

  Please contact our office during our regular business hours to request an appointment at Hilltop Dental.

• How do I cancel or reschedule an appointment?

  To reschedule or cancel an appointment, please call us. Please let us know about any change of plans as soon as you can, so the space in our schedule can be offered to another patient.

• Do you offer appointment reminders?

  In the days before your appointment, you will receive automated phone appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.

• Does your clinic have on-site parking?

  Yes, free on-site parking is available for all our patients. We are also easily accessible by public transit.

Insurance & Payment

At Hilltop Dental, we'll help you navigate our options for payment and direct billing to insurance companies so you can focus on getting the dental care you need.

• Direct Billing to Insurance

  We offer direct billing for most insurance plans. We can submit your insurance claim electronically for your convenience, which results in faster reimbursement and avoids the need for signatures and paper work.

  Our administrative team can assist you in understanding your insurance plan better and can send estimates to your insurance company before starting any major treatments. 

  Please bring your insurance plan information with you to your appointment.

• Payment Methods

  You are responsible for any co-pays or amounts not covered by insurance. These must be paid at the time of your appointment.

  We accept payment by cash, debit or most major credit cards.
